Benefits Transmittal

Purpose
Requesting a benefits transmittal from Clover Park Technical College

Overview

When requesting a benefits transmittal with Human Resources and Payroll please attach the Benefits Transmittal Form in the ticket along with some of the employee's information. 

After creating a ticket you will receive a link to all of your open tickets where you can track their progress. 

  • After creating a ticket you may receive an email requesting additional information if your ticket requires more detailed information to complete. 
  • Once your ticket/request has been completed by Payroll the ticket will be updated and you will receive an email with the completed Benefits Transmittal form which will be sent to the email that was provided in the form.

To request a verification of employment please select the Request Benefits Transmittal button on the right-hand side of the screen.
